Your First Appointment

Please arrive 15 minutes so that we can complete your initial paperwork.

Please bring the following:

• The Client Information and Payment Form (PDF)*
• Your pet's current prescriptions and/or over-the-counter medications, vitamins and supplements (including dose and frequency)
• Any test results, x-rays, medical and surgical records
• Any other relevant information about your pet's health and behavior

Payment Information

We accept payment by Visa, MasterCard, American Express, Discover, and cash.
No personal checks, please.
